Hemimegalencephaly and Intractable Epilepsy: Benefits of Hemispherectomy

Abstract
Summary: We observed 4 children with hemimegalencephaly, (3 boys, 1 girl aged 3–7 years). One child had a linear sebaceous nevus. All patients had a similar clinical, EEG, and neuroradiologic pattern. All patients had mac‐rocrania, hemiparesis, hemianopsia, and psychomotor retardation of variable degree. All cases had an epileptic syndrome with onset during the first days or the first months of life. The seizures were consistently similar: partial motor seizures, generally hemiclonic and asymmetric brief tonic seizures, in series, involving predominantly one side of the body, contralateral to the cerebral damage. The EEG was initially characterized by a hemi‐hypsarrhythmia and afterward, over the malformed hemisphere, by a rather high‐frequency background activity associated with almost continuous transients of spikes, sharp waves, and spike and waves that progressively involved the contralateral hemisphere. Hemispherectomy was performed in two children with sudden and complete recovery from seizures and improvement in psychomotor development. Macroscopic examination was consistent with the computed tomography (CT) scan and magnetic resonance imaging (MRI) showing pachygyria and poorly represented white matter. Histologic examination showed the characteristic absence of cortical neuronal layering, and the presence of giant neurons, neuronal heterotopias, and gliosis. For hemispherectomy to be successful, we believe prolonged EEG monitoring is needed to verify that no seizures are originating in the normal hemisphere. RÉSUMÉ Les auteurs ont observé 4 enfants présentant une hémimé‐galencéphalie (3 gargons, 1 fille, âgés de 3 à 7 ans). Un cas pré‐sentait un naevus sébacé linéaire. Tous les patients présentaient un tableau clinique EEG et neuroradiologique similaire: macro‐crânie, hémiparésie, hémianopsie et retard psychomoteur de degrés variables. Tous présentaient un syndrome eapileptique ayant débuté dans les premiers jours ou dans les premiers mois de la vie. Les crises etaient assez stéréotypées: crises partielles mo‐trices, généralement hémicloniques, crises toniques asymétriques brèves en series, intéressant de façon prédominate l'hémicorps contro‐latéral à la lésion cérébrale. EGG eatait caractérisé initialement par une hémi‐hypsarythmie, avec apparition secondaire, au niveau de hémisphere malformé une activité de fond de grande amplitude associée à des anomalies subcontinues à type de pointes, éléments angulaires et de pointes‐ondes, qui se sont étendues progressivement vers autre hémisphére. Une hémisphérectomie a été pratiquée chez 2 enfants et a été suivie un arreCt brutal et complet des crises et une amélioration du développement psychomoteur. examen macroscopique a confirmé les données de la tomodensitométrie et de TRM: pachygyrie et pauvreté en substance blanche. examen histologique a mis en évidence absence caractéristique organisation corticale et la présence de neurones géants, hét‐érotopies neuro nales et de gliose. Afin assurer le succès de hémisphérectomie, les auteurs recommandent la pratique des enregistrements EEG de longue durée qui permettent de vérifier qu'il n'y a pas de crises prenant leur origine sur hémisphère contralatéral. RESUMEN Hemos observado cuatro niños con hemimegalencefalia (3 varones y 1 hembra). Las edades oscilaban entre 3 y 7 años. Un caso tenía un nevus sebáceo linear. Todos los pacientes presen‐tarón un patron semejante tanto clinico como electroencefa‐lografico o neurorradiológico. Todos los casos tenían macrocra‐nia, hemiparesia, hemianopsia y retraso psicomotor de grado variable. Todos los enfermos tenían un síndrome epiléptico de comienzo en los primeros días o durante el primer mes de vida. Los ataques fueron consistentemente semejantes: ataques pa‐ciales motores, ataques tonicos breves, hemiclónicos y general‐izados o asimétricos que se presentaban en series afectando fundamentalmente el lado del cuerpo contralateral a la lesión cerebral. El EEG se caracterizó, inicialmente, por una hemihip‐sarritmia y, posteriormente, por una actividad de fondo de alta frecuencia originándose en el hemisferio malformado asociada a brotes de puntas casi contfnuos, ondas agudas y complejos punta‐onda que progresivamente afectaron al hemisferio contralateral. Se practiceó una hemisferectomía en dos niños con rápida y completa recuperación de los ataques y una majoría en el de‐sarrollo psicomotor. El exámen microscópico mostró hallazgos semejantes a los del CT y de la MR con reducido tamaño de las circunvoluciones y substancia blanca pobremente representada. El exámen histológico reveló la característica ausencia neuronal en las diferentes capas de la corteza y la presencia de neuronas gigantes, heterotopias neuronales y gliosis. Para que una hemiferectomía dé resultados satisfactorios los autores piensan que se necesita una monitorización prolongada del EEG para verificar que ningún ataque se origina en el hemisferio normal. ZUSAMMENFASSUNG Wir beobachteten 4 Kinder mit Hemimegalenzephalie (3 M, 1 F im Alter zwischen 3 und 7 Jahren). Ein Kind zeigte einen linearen Naevus sebaceus. Alle Patienten zeigten ein aUhnliches klinisches, elektroenzephalographisches und neuroradiologis‐ches Muster. Alle Patienten hatten eine Makrokranie, Hemi‐parese, Hemianopsie und psychomotorische Retardierung unter‐schiedlichen Ausmaßes. Ebenso zeigten alle Patienten ein epi‐leptisches Syndrome mit Beginn in den ersten Lebenstagen oder Monaten. Die Anfälle waren durchgehend ähnlich: Partial‐motorische Anfälle, generalisierte Hemikloni und kurze, asym‐metrische to nische Anfälle, die in Serien auftraten und über‐ wiegend die zur zerebralen Stöning kontralaterale Körperseite betrafen. Das EEC war zu Beginn durch eine Hemihypsarrhyth‐mie gekennzeichnet und später über der mißgebildeten Hemisphäre durch schneile Hintergrundaktivität zusammen mit nahezu kontinuierlichen Spikes, Sharp Waves und Spike Waves, die zunehmend auch die kontralaterale Hemisphäre miteinschlos‐sen. Zwei Kinder wurden hemisphärektomiert und wurden so‐fort komplett anfallsfrei mit Besserung der psychomotorischen Entwicklung. Der makroskopische Befund stimmte mit dem CT‐und MR‐Befund überein und zeigte eine Pachygyrie und nur mäßig ausgeprägte weißes Substanz. Die histologische Untersuc hung zeigte das charakteristische Fehlen von kortikalen, neuron‐alen Schichten sowie Riesenneuronen, neuronale Heterotopien und Gliose. Um eine erfolgreiche Hemisphärektomie durch‐führen zu können, ist unserer Meinung nach ein längerdauerndes EEG‐Monitoring notwendig, um aicherzustellen, daß keine An‐fälle von der gesunden Hemisphäre ausgehen.
