Persistierende bilaterale isolierte Sakroiliitis

Abstract

"HLA-B27 positive, low back pain and sacroiliitis" represent a cluster of signs and symptoms often misinterpreted as "ankylosing spondylitis". Compared to ankylosing spondylitis persistent bilateral isolated sacroiliitis shows a considerably different potential of prognosis, concerning planning and quality of life (private, profession), medical and physiotherapeutic regimens and last but not least the general costs of treatment. Problems of nomenclature, advantage and disadvantage of existing diagnostic criteria (Rome, New York, modified New York) are discussed. These criteria perpetuate a specific clinical appearance traditionally - often wrongly - regarded as typical of ankylosing spondylitis and dont allow differentiation to persistent bilateral isolated sacroiliitis. The frequency of low back pain combined with population-specific frequency of HLA-B27 on one side - prevalence of white hospitalized patients with ankylosing spondylitis (0.05-0.25%) compared to the prevalence of bilateral isolated sacroiliitis in population studies (1-1.7%) or in AS-collectives (≥4.5% after a disease course of appr. 5 years) on the other side are convincing arguments for a disease (persistent) "bilateral isolated sacroiliitis". This disease has to be differentiated from ankylosing spondylitis and diseases related to ankylosing spondylitis. Still not clear and subject to further examination is the amount of time until the bilateral isolated sacroiliitis will take over from the diagnosis ankylosing spondylitis. Zusammenfassung Die Befundkonstellation ,,HLA-B27 nachweisbar, tiefer Kreuzschmerz und Sakroiliitis" wird sehr häufig als beweisführend für die Diagnose ,,Spondylitis ankylosans" angesehen. Eine persistierend bilaterale isolierte Sakroiliitis (BIS) hat jedoch verglichen mit einer Spondylitis ankylosans ein erheblich differentes diagnoseimmanentes Prognosepotential, das berufliche und private Lebensplanung, medikamentöse und krankengymnastische Therapiestrategien und die gesamtfinanziellen Kosten der Therapie betrifft. Dargestellt werden nomenklatorische Probleme. Vor- und Nachteile bisheriger Diagnosekriterien (Rom, New York, modifizierte New York-Kriterien) - funktioneller, klinischer und radiologischer Art werden diskutiert. Sie alle perpetuieren - häufig falsch - den ursprünglichen Inhalt der Diagnose Spondylitis ankylosans und erlauben die Abgrenzung zur bilateralen isolierten Sakroiliitis nicht. Die Häufigkeit tiefsitzender Kreuzschmerzen in Kombination mit der bevölkerungsspezifischen Häufigkeit von HLA-B27 einerseits - die Sp.a.-Prävalenz weißer Hospitalisierter (0,05-0,25%) verglichen mit der von BIS in Bevölkerungsstudien (1-1,7 %) oder bei ,,Sp. a.-Patienten" (>4,5% nach ca. 5 Jahren Verlauf) legen andererseits den Schluß eines eigenen Krankheitsbildes - der bilateralen isolierten Sakroiliitis nahe. Diese Krankheit muß - der erwähnten Unterschiede wegen - von der Sp. a. und Sp.a.-verwandten Krankheiten getrennt werden. Noch unklar und weiteren Untersuchungen vorbehalten bleibt der zeitliche Rahmen, in dem die persistierende bilaterale isolierte Sakroiliitis die Diagnose Spondylitis ankylosans ablösen muß.
