Intima-media thickness (IMT) is an important indicator of endothelial function. The aim of the present study was to investigate the predictive accuracy of carotid and cavernous Doppler ultrasound findings for discriminating patients with vasculogenic ED. METHODS Fifty patients applying to our clinic with the complaints of ED were included in the study. The patients underwent detailed medical and sexual history, physical examination, routine biochemical blood tests and serum lipid profile. B-mode ultrasound of bilateral carotid arteries were performed and peak systolic velocity (PSV), end-diastolic velocity (EDV) and IMT values were measured. After papaverine injection, corresponding values of cavernous arteries were obtained by penile duplex ultrasonography (PDU). Of total 50 patients, 29 (58%) had had vasculogenic ED according to PDU findings and they were included in organic ED group. The remaining 21 (42%) patients having normal PDU values were contained in psychogenic ED group. Two groups were compared regarding carotid and cavernous Doppler findings (Table 1). Mann-Whitney U test was applied and p values ¡(0.05 were considered as significant. Table 1. Patient characteristics and ultrasound data. Values were expressed as mean ± standard deviation (min-max). Organic ED (n=29) Psychogenic ED(n=21) p value BMI (kg/m2) 27,4±4,2(19,6–36,3) 27,1±2,7(21,1–31,8) 0,768 Age (years) 52,2±11,0(30–77) 48,7±8,3(31–68) 0,156 IIEF score 10,6±5,3(5–20) 17,3±2,6(13–21) <0,001 Carotid Arteries IMT (mm) 0,83±0,20(0,40–1,30) 0,79±0,19(0,43–1,13) 0,601 PSV (cm/second) 75,1±19,7(40,0–115,0) 94,0±12,4(75,0–122,0) 0,001 EDV (cm/second) 23,8±6,8(11,0–42,0) 34,0±7,9(21,0–51,0) Cavernous Arteries IMT (mm) 0,21±0,09(0,10–0,40) 0,14±0,10(0,03–0,33) 0,012 PSV (cm/second) 21,7±6,9(7,0–33,0) 49,8±21,3(30,0–130,0) <0,001 EDV (cm/second) 3,8±3,5(0–10,0) 0,2±1,1(0–5,0) <0,001 ED, Erectile Dysfunction; IIEF, International Index Of Erectile Function; BMI, Body Mass Index; IMT, Intima-Media Thickness; PSV, Peak Systolic Velocity; EDV, End-Diastolic Velocity. RESULTS Carotid and cavernous PSV and EDV values of two groups were significantly different (p<0.001). There was also a significant difference between the groups regarding cavernousal IMT (p=0.012). However, we did not observe the same difference taking into consideration carotid IMT (p=0.601) (Table 1). CONCLUSIONS Carotid artery PSV and EDV successfully mirror respective measurements in cavernous artery. However, carotid IMT fails to demonstrate such a correlation. Moreover, a novel doppler parameter, cavernous IMT seems promising as an additional tool in evaluation of cavernosal function by PDU. © 2011 by American Urological Association Education and Research, Inc.FiguresReferencesRelatedDetails Volume 185Issue 4SApril 2011Page: e529-e530 Advertisement Copyright & Permissions© 2011 by American Urological Association Education and Research, Inc.MetricsAuthor Information Cevdet Serkan Gökkaya Ankara, Turkey More articles by this author Binhan Kagan Aktas Ankara, Turkey More articles by this author Ugur Toprak Ankara, Turkey More articles by this author Sedat Yahsi Ankara, Turkey More articles by this author Suleyman Bulut Ankara, Turkey More articles by this author Cuneyt Ozden Ankara, Turkey More articles by this author Ali Memis Ankara, Turkey More articles by this author Expand All Advertisement Advertisement Loading ...
