The Time Relationships of Migraine and Cluster Headache When Occurring in the Same Patient

SYNOPSIS Of 1,688 patients with migraine or cluster headache, 16 (one percent) had a history of both. In 14 of the 16 patients, migraine occurred first and was either replaced by cluster headache (7 patients) or continued during the years of cluster headache (7 patients). In one patient, migraine and cluster headache began at the same time and in one patient, cluster headache preceded migraine. A review of the literature reveals a similar sequence of migraine almost always preceding cluster headache when the two occur in the same patient. The relationships between migraine and cluster headache are discussed. In rare instances, cluster headache appears to evolve from migraine.
