症例は53歳，女性．人間ドックの胸部CTにて前縦隔腫瘍を指摘され入院となった．術前診断では，胸腺腫または胸腺嚢胞を疑い，手術を施行した．手術は胸腔鏡補助下に腫瘍を摘出した．病理診断は胸腺内発生の気管支原性嚢胞であった．気管支原性嚢胞が前縦隔胸腺内に発生することは稀であり報告する．
