Abstract

Standardtherapie der Lungentuberkulose ist die Vierfachtherapie mit Isoniazid, Rifampicin, Pyrazinamid und Ethambutol über zwei Monate, gefolgt von einer Zweifachtherapie mit Isoniazid und Rifampicin über weitere vier Monate. Auf Ethambutol kann jedoch verzichtet werden, wenn eine nachgewiesene Empfindlichkeit gegenüber Isoniazid, Rifampicin und Pyrazinamid besteht. Diese Therapiestrategie hat sich auch für die meisten Formen der extrapulmonalen und nichtpulmonalen Formen der intrathorakalen Tuberkulosen bewährt und sollte bis auf Fälle einer Beteiligung des zentralen Nervensystems (zwölf Monate Therapiedauer) ebenfalls als Standard angesehen werden. Basis dieser Empfehlung ist, dass zum einen von einer vollen Sensibilität des Erregers gegenüber den verwendeten Substanzen, zum anderen von ihrer guten Verträglichkeit über den gesamten Therapiezeitraum ausgegangen werden kann. Kann oder können aus Resistenz- oder Verträglichkeitsgründen eines oder mehrere der oben genannten Medikamente nicht eingesetzt werden, so verlängern sich die Therapiezeiten zum Teil beträchtlich. Nach gesicherter Diagnose einer latenten tuberkulösen Infektion und entsprechender Indikationsstellung kann bei anamnestisch wahrscheinlicher Isoniazid-Sensibilität eine neunmonatige präventive Therapie mit maximal 300mg Isoniazid pro Tag erfolgen. Standard medical treatment of pulmonary tuberculosis should contain the four substances isoniazid, rifampicin, pyrazinamide, and ethambutol for two months, followed by isoniazid and rifampicin for four months. Ethambutol is not necessary if the strain is known to be fully susceptible to isoniazid, rifampicin and pyrazinamide. This regimen is also the standard approach for most extrapulmonary and non-pulmonary intrathoracic types of tuberculosis with the exception of tuberculosis of the central nervous system (treatment duration twelve months). The recommendation is based on both drug susceptibility of the bacteria and on the absence of serious side effects due to the drugs used. If, due to drug resistance or side effects, one ore more of the above-mentioned substances cannot be used, the treatment period has to be increased substantially. Following a clear diagnosis of latent tuberculosis infection and probable isoniazid susceptibility (history) a nine-month preventive therapy using isoniazid (max. 300mg/d) can be offered.
