Abstract
Aplasia cutis congenita (ACC) is a rare malformation involving the scalp and sometimes the underlying tissues. It may occur as an isolated defect or be combined with congenital malformations and there is often a family background. The mortality rate is high, with infectious and vascular complications. We report our experience based on the treatment of 5 ACC of the scalp in children, where a family pathology was found. Controlled healing is the best approach for us, even for the serious forms. After the acute period, treatment is based on skin expansion and must be followed by genetic screening of the family. Résumé L'aplasie cutanée congénitale est une malformation rare pouvant atteindre le scalp et parfois les tissus sous-jacents. Elle peut être isolée ou associée à des malformations congénitales, souvent dans un contexte familial. La mortalité y est élevée, à l'occasion de complications vasculaires ou infectieuses. Les auteurs rapportent leur expérience dans la prise en charge de 5 nouveaux-nés atteint d'ACC du vertex dans un contexte familial. Le traitement par cicatrisation dirigée semble être la meilleure attitude, même dans les formes sévères. Après la période aiguë, enquête familiale et expansion cutanée complète la prise en charge Resumen La Aplasia Cutis Congénita (ACC) es una malformación rara del cuero cabelludo que afecta a veces a los tejidos subyacentes. Puede presentarse aisladamente o asociada a otras malformaciones habiendo con frecuencia antecedentes familiares. La mortalidad es alta debido a complicaciones infecciosas y vasculares. Presentamos nuestra experiencia basada en el tratamiento de 5 casos de ACC en niños en quienes se daba un contexto familiar. La cicatrización controlada es, para nosotros, la mejor opción incluso en los casos graves. Tras el periodo agudo el tratamiento se basa en la expansión tisular y debe acompañarse de una encuesta genética familiar. Zusammenfassung Die Aplasia cutis congenita (ACC) ist eine seltene Missbildung der Kopfhaut, die ebenfalls die tiefer liegenden Gewebe mit einbeziehen kann. Sie kann sowohl isoliert als auch in Kombination mit anderen kongenitalen Missbildungen auftreten, oft beobachtet man eine familiäre Häufung. Die Mortalitätsrate ist aufgrund der infektionsbedingten und vaskulären Komplikationen hoch. Unser Bericht basiert auf 5 Fällen von ACC bei Kindern, die wir in unserem Zentrum behandelt haben und bei denen wir eine familiäre Häufung feststellen konnten. Unserer Auffassung nach ist die Methode der Wahl eine kontrollierte Wundheilung, und zwar auch bei der schweren Form. Nach der akuten Phase wird die Behandlung mit einer Hautexpansion fortgeführt und wir empfehlen eine genetische Untersuchung der Familienmitglieder.
