Neovaginectomy Following Vaginoplasty in a Male-to-Female Transgender Patient

Abstract
In Brief Sex reassignment surgery has been used in select patients with gender identity disorder. Surgical interventions in male-to-female transexuality include penectomy, orchiectomy, construction of a neovagina, and other nongenital procedures, such as breast augmentation and facial feminization surgery. Approach to care including psychotherapy, hormonal and surgical therapy promotes psychologic well being and self-fulfillment. While patients and surgeons tend to emphasize esthetic and functional outcomes of sexual reassignment surgery, 46% of patients report never using their neovagina for coitus. We present the case of a 52-year-old male-to-female transsexual patient who presented 6 years after initial operation involving vaginoplasty using the Stuttville technique. Now living again as a man, he requested colpectomy of the neovagina in preparation for transgender reversal. Transgender reversal surgery involving neovaginectomy in male-to-female transgender has been very rarely reported in the literature. Uncomplicated excision of the neovagina is achieved with strict adherence to vaginal surgery principles. A case of colpectomy after initial vaginoplasty in a male-to-female transgender reversal describing surgical techniques and brief review of literature on the subject.
