Hemifacial spasm and surgical treatment (microvascular decompression)

Abstract

Hemifacial spasm is a common disease however there are some facial movement disorders which have resemblance to hemifacial spasm. Misdiagnosis of hemifacial spasm can mislead to wrong treatment. Hemifacial spasm is characterized by involuntary contractions of muscle groups innervated by ipsilateral facial nerve. The symptoms usually start around the eyes then progressing downward to the cheek, mouth and neck. Its prevalence is 9.8 per 100,000 patients with mean age of onset at 44 – 52 years. The pathophysiology of hemifacial spasm is caused by pathology which compresses the facial nerve at facial nerve root exit zone (FNREZ) such as vessel loops, tumors, etc. Clinical signs, and symptoms together with imaging modalities such as electromyography (EMG), and magnetic resonance imaging (MRI) are useful to differentiate hemifacial spasm from other facial movement disorders and can also identify the pathology for preoperative planning. Botulinum neurotoxin injection is a standard medical management for hemifacial spasm which has low risk but provides limited symptomatic relief. Microvascular decompression (MVD) is the only curative treatment for hemifacial spasm that provides lasting symptomatic relief with low rate of complications because of improvised microsurgical techniques, microneurosurgical instruments and also intraoperative neuromonitoring. Key words: Hemifacial spasm, Treatment, Microvascular decompression โรคใบหน้ากระตุกครึ่งซีก และการรักษาด้วยการผ่าตัดปรีดิ์ นิมมานนิตย์ภาควิชาศัลยศาสตร์ คณะแพทยศาสตร์ มหาวิทยาลัยธรรมศาสตร์ บทคัดย่อ โรคใบหน้ากระตุกครึ่งซีก (hemifacial spasm) เป็นโรคที่พบได้บ่อยในเวชปฏิบัติทั่วไป หากแต่ยังมีโรคอีกจำนวนหนึ่งที่มีอาการคล้ายคลึงกับโรคนี้ ทำให้การวินิจฉัยที่ไม่ถูกต้อง นำไปสู่การรักษาที่ไม่ถูกต้อง อาการของโรค hemifacial spasm คือ มีการกระตุกใบหน้าซีกเดียว ของกล้ามเนื้อที่เลี้ยงโดยเส้นประสาทสมองคู่ที่ ๗ (facial nerve) ข้างเดียวกัน โดยมักจะเริ่มกระตุกบริเวณรอบดวงตา แล้วกระจายลงล่างไปสู่กล้ามเนื้อบริเวณแก้ม ปาก และคอ มีอุบัติการณ์การเกิดโรคประมาณ ๙.๘ คนต่อประชากร ๑๐๐,๐๐๐ คน และเกิดในผู้ป่วยอายุเฉลี่ยประมาณ ๔๔ - ๕๒ ปี สาเหตุการเกิดโรคนี้เกิดจากการที่มีพยาธิสภาพ เช่น หลอดเลือด หรือเนื้องอก กดทับลงบน facial nerve บริเวณที่เพิ่งยื่นออกมาจากแกนสมอง (facial nerve root exit zone; FNREZ) การวินิจฉัยแยกโรคนี้ออกจากโรคอื่น ที่มีอาการคล้ายกัน ทำได้โดยอาศัย อาการ และอาการแสดง ร่วมกับการตรวจคลื่นไฟฟ้ากล้ามเนื้อ (electromyography; EMG) และเอกซเรย์คลื่นแม่เหล็กไฟฟ้าสมอง (magnetic resonance imaging; MRI) จะสามารถบอกถึงพยาธิสภาพที่ทำให้เกิดโรคได้อีกด้วย การรักษาโรค hemifacial spasm ด้วยการฉีด botulinum neurotoxin (BoNT injection) ถือเป็นวิธีมาตรฐานของการรักษาโดยการไม่ผ่าตัดที่มีความเสี่ยงต่ำ แต่ให้ผลการรักษาที่ไม่ดีนัก และต้องฉีดซ้ำทุกๆ ๓ - ๖ เดือน การรักษาด้วยวิธีผ่าตัดแยกหลอดเลือดกดทับ ออกจาก FNREZ (microvascular decompression; MVD) เป็นการรักษาวิธีเดียว ที่สามารถทำให้ผู้ป่วยหายขาดจากโรคนี้ได้ เนื่องจากเป็นการรักษาที่ต้นเหตุ ทั้งมีผลข้างเคียงจากการผ่าตัดต่ำ เนื่องจากในปัจจุบันเทคนิคการผ่าตัด เครื่องมือผ่าตัด และเครื่องมือติดตามระหว่างการผ่าตัดมีการพัฒนาเป็นอย่างมาก คำสำคัญ: โรคใบหน้ากระตุกครึ่งซีก, การรักษา, การผ่าตัดรักษา
