Abstract

บทคดยอ Myelodysplastic syndrome (MDS) เปนกลมโรคของเซลลตนกำเนดเมดเลอดทมเซลลเมดเลอดลดลงและมความผดปกตในการสรางเมดเลอดรวมกบความผดปกตของ epigenetics การรกษา MDS มคาใชจายสงและบางการรกษากไมสามารถทำไดในหลายพนทของประเทศไทย รายงานผปวยนแสดงผลการรกษาผปวย 4 รายดวย valproic acid แบบ controlled release อยางเดยว ปรบใหระดบความเขมขนในกระแสเลอด 50-100 μg/mL ซง valproic acid มฤทธเปน histone deacetylase inhibitor อาจทำใหการทำงานของยนทปกตดขน และ เปนยาทราคาไมแพง มผลขางเคยงนอย และสามารถรบประทานได พบวาผปวยทง 4 ราย ตอบสนองตอการรกษาภายใน 1-6 เดอน ดงนน valproic acid จงอาจพจารณาเปนทางเลอกหนงในการรกษาผปวย MDS Key Words : Myelodysplastic syndrome, Valproic acid, การรกษา, Histone deacetylase inhibitor Abstract MDS is a group of disorders of hematopoietic stem cells characterized by cytopenia and ineffective hematopoiesis associated with epigenetic abnormalities.Treatments of MDS are costly and may not be available in some areas of Thailand.This is a report of the uses of single-agent controlled-release valproic acid (target serum concentration 50-100 μg/mL) in 4 cases of MDS; refractory anemia (2) and refractory cytopenia with multi-lineage dysplasia (2).Valproic acid, a histone deacetylase inhibitor, that may enhance normal gene expression, is inexpensive, non-toxic and can be administered orally.After the valproic acid treatments, there were hematological improvements within 1-6 months in all 4 patients.Therefore, valproic acid alone may be a good alternative for the treatment of MDS.
