THE REPORT IS THE FOURTH FINAL REPORT ISSUED SINCE ACCIDENT INVESTIGATION RESEARCH WAS INITIATED IN FEBRUARY 1968. METHODOLOGY AND FINDINGS OF MULTIDISCIPLINARY ACCIDENT INVESTIGATION OF 43 CASES ARE DISCUSSED AND CONCLUSIONS AND RECOMMENDATIONS ARE DERIVED FROM THE ANALYSES OF THESE CASE STUDIES. /AUTHOR/
