Development of coronary arteries in transposition of the great arteries

The study of the origin and branching pattern of the coronary arteries in transposition of the great arteries (TGA) needs updating. A thorough investigation of all variations has been documented [12], but it lacks the special emphasis that would make it relevant for present clinical use. This latter subject refers to the arterial-switch operation in which the main procedure for surgical correction of TGA is performed at the level of the great arteries [7]. An essential part is the feasibility of surgical relocation of the coronary arteries to the newly formed aortic orifice, being originally the pulmonary orifice. The former classification of the coronary arterial origin was far too complicated to be able to sort out which type of coronary arterial pattern could be switched and in which cases it would be wiser to develop an alternative technique or to stay with the already well-known Mustard or Senning procedures.
